Dear Sir:

 

Alicia Oceguera is an Apprentice at the Stickney Water Reclamation Plant. On January 9, 2012, she sustained injuries to her low back while shoveling snow and then slipping. At the time of the injury, Ms. Oceguera was a Maintenance Laborer A. Ms. Oceguera was initially seen at the industrial clinic on January 9, 2012. She was eventually diagnosed with a herniated disc at L5-S1. She treated conservatively with medication and physical therapy. She was disabled from work effective January 12, 2012. She was able to return to work on February 15, 2012. 

 

Ms. Oceguera received 4.80 weeks of lost time benefits totaling $4,199.52.   

 

Ms. Oceguera filed an Application of Adjustment of Claim with the Illinois Workers’ Compensation Commission (IWCC) through her counsel. Subject to the approval of the IWCC, this case can now be settled for a total of $16,000.00, representing approximately 4.59% loss of use of the person as a whole. This settlement will close out future lost time and medical benefits associated with this injury.

 

The Director of Human Resources believes this settlement is in the best interest of the District and requests payment of that sum be approved and she be authorized to execute such documents as may be necessary to effect the settlement.
