Authority to amend Board Order of April 5, 2012, authorizing the issuance of a 1-year permit to K-Five Construction Corporation, Inc. to access District Main Channel Parcels 29.01, 30.01, 30.03, 31.01, 31.02 and 31.03 between LaGrange Road and Willow Springs Road to repair areas of Interstate 294 known as the Mile Long Bridge in Willow Springs, Illinois to reflect the name of the permittee as K-Five Construction Corporation, Inc./Lorig Construction Company, a Joint Venture.

Dear Sir:
On April 5, 2012, the Board of Commissioners duly authorized the issuance of a 1-year permit to K-Five Construction Corporation, Inc. ("K-Five") to access District Main Channel Parcels 29.01, 30.01, 30.03, 31.01, 31.02 and 31.03 between LaGrange Road and Willow Springs Road to repair areas of Interstate 294 known as the Mile Long Bridge in Willow Springs, Illinois, Agenda Item No. 88, File No. 12-0549.
The initial permit request was from K-Five. After the permit was issued, K-Five advised that it obtained the contract from the Illinois State Toll Highway Authority as a joint venture with Lorig Construction Company. As a result of the joint venture, K-Five has requested the permit be issued in the name of the joint venture.
It is requested that the Board Order of April 5, 2012 be amended from authorizing the issuance of a 1-year permit to K-Five Construction Corporation, Inc. to access District Main Channel Parcels 29.01, 30.01, 30.03, 31.01, 31.02 and 31.03 between LaGrange Road and Willow Springs Road to repair areas of Interstate 294 known as the Mile Long Bridge in Willow Springs, Illinois, to reflect the name of the permittee as K-Five Construction Corporation, Inc./Lorig Construction Company, a Joint Venture.
