Dear Sir:

 

Erin Keohane is an Administrative Specialist at the Stickney Reclamation Plant. On July 23, 2021, the employee reported slipping on a puddle of bug spray and striking a metal cage, injuring her left knee. The employee went to the industrial clinic on July 23, 2021, for injuries to her left knee.  She treated conservatively with medication and physical therapy; however, her left knee did not improve. On October 15, 2021, she underwent surgery to repair her left knee and was disabled from work at this time. After a course of physical therapy, she returned to work on December 7, 2021.

 

Ms. Keohane was paid 7.40 weeks of lost time benefits totaling $7,602.51.        

 

Ms. Keohane filed an Application of Adjustment of Claim with the Illinois Workers’ Compensation Commission (IWCC) through her counsel. Subject to the approval of the IWCC, this case can now be settled for a total of $30,221.80, representing approximately 15% loss of use of the left leg. This settlement will close out future lost time and medical benefits associated with this injury.

 

The Director of Human Resources believes this settlement is in the best interest of the District and requests payment of that sum be approved and she be authorized to execute such documents as may be necessary to effect the settlement.
