Dear Sir:

 

Daniel Gierut worked as a Pipefitter at the Stickney Water Reclamation Plant. He retired from the District effective May 1, 2024. On September 15, 2022, the employee reported that while walking in Digester Area B, he tripped on an abandoned pipe sticking up from the floor, grabbed onto an electrical box to catch himself, and jammed his left fingers. Mr. Gierut sought treatment at the Industrial Clinic on September 20, 2022, and treated conservatively with a splint for his left hand. On September 29, 2022, he was diagnosed with a fracture of the left middle finger and the left ring finger. He was taken off work on September 30, 2022.  The employee returned to work on October 31, 2022.

 

Mr. Gierut was paid a total of 4.2 weeks of lost time benefits totaling $5,857.15.

 

Subject to the approval of the IWCC, this Pro-Se case can be settled for $14,596.04, representing approximately 22.5% loss of each finger. This settlement will close out any future lost time and medical benefits associated with this injury.

 

The Director of Human Resources believes this settlement is in the best interest of the District and requests payment of that sum be approved and he be authorized to execute such documents as may be necessary to effect the settlement.
