Dear Sir:

 

Brian Kalata is an Engineering Technician V, working at the Stickney Water Reclamation Plant. On June 28, 2017, the employee reported that he sustained an injury to his left shoulder after being involved in a work vehicle accident while travelling westbound on 119th Street in Palos Park, Illinois. The employee sought medical treatment immediately at the industrial clinic on June 28, 2017. He was given some medications with the understanding that his injury would resolve on its own. However, the employee continued to experience pain and sought treatment with a specialist on January 11, 2019. After an MRI was performed it was determined that he required surgery. On May 14, 2019, he underwent surgery on his left shoulder. He was placed on disability at that time. Following the surgery, the employee underwent a course of physical therapy. He was able to return to work on July 2, 2019.  

 

He was paid a total of 7 weeks of lost time benefits totaling $9,162.19. 

 

Subject to the approval of the IWCC, this Pro-Se case can be settled for $46,510.80, representing approximately 12% loss of use of the person as a whole. This settlement will close out any future lost time benefits as well as any future medical benefits associated with this injury.

 

The Director of Human Resources believes this settlement is in the best interest of the District and requests payment of that sum be approved and she be authorized to execute such documents as may be necessary to effect the settlement.
