Dear Sir:
Miguel Negron II is a Maintenance Laborer A Shift at the Stickney Water Reclamation Plant. On August 15, 2022, Mr. Negron reported that he was pulling weeds out of a tank with a large rake, swinging it over his shoulder to the ground, and injured his left shoulder. The employee was seen at the Industrial Clinic on August 15, 2022, and was diagnosed with left shoulder strain. Mr. Negron had an MRI of the left shoulder on August 24, 2022, that revealed an interstitial tear of the supraspinatus and infraspinatus tendons. The employee had left shoulder surgery on September 20, 2022. He was off work beginning on September 20, 2022, and returned to work on November 3, 2022.
Mr. Negron was paid 6.4 weeks of lost time benefits totaling $7,356.54.
Subject to the approval of the Illinois Workers’ Compensation Commission, IWCC, this Pro Se case can be settled for $59,881.20 representing approximately 12% loss of person as a whole. This settlement will close out any future lost time and medical benefits associated with this injury.
The Director of Human Resources believes this settlement is in the best interest of the District, and requests payment of that sum be approved and he be authorized to execute such documents as may be necessary to effect the settlement.
