COMMITTEE ON JUDICIARY

Mr. Brian A. Perkovich, Executive Director

Title

Authority to settle the Illinois Workers’ Compensation Claim of Andrew Soulian vs. MWRDGC, Claim Number W002566890 in the sum of $48,831.20, Account 901-30000-601090

Body

Dear Sir:

Andrew Soulian is an Architectural Ironworker working at the Stickney Reclamation Plant. On July 18, 2018 the employee reported that he sustained an injury to his left shoulder after lifting heavy steel plates while performing his job. He was initially seen at the industrial clinic on July 18, 2018. He was initially treated conservatively with medication and physical therapy. Mr. Soulian underwent a procedure to repair his left shoulder on October 18, 2018 and was disabled from work at this time. Following the surgery, the employee underwent a course of physical therapy. He was able to return to work on February 1, 2019.

He was paid a total of 16.4 weeks of lost time benefits totaling $20,739.16.

Subject to the approval of the Illinois Workers’ Compensation Commission (IWCC), this Pro-Se case can be settled for $48,831.20, representing approximately 12% loss of the person as a whole. This settlement will close out any future lost time benefits as well as any future medical benefits associated with these injuries.

The Director of Human Resources believes this settlement is in the best interest of the District and requests payment of that sum be approved and she be authorized to execute such documents as may be necessary to effect the settlement.

Requested, Beverly K. Sanders, Director of Human Resources, BKS:RAJ:RG
