Dear Sir:
 
Alicia Gibbs was a Maintenance Laborer at the Stickney Water Reclamation Plant.  She was injured on June17, 2010, during the course of performing her duties.  Ms. Gibbs sought treatment with her own physician and was diagnosed with DeQuervains tenosynovitis of the right wrist and thumb.  She received an injection as part of her treatment.  Ms. Gibbs missed a total of four working days.
 
On June 7, 2011, Ms. Gibbs sustained an injury to her right wrist while assisting in a clean-up of a spill.  She went to her doctor and was noted to have right DeQuervains tenosynovitis with dorsal radial sensory nerve neuritis, right hand numbness and tingling, and right thumb MP joint pain with underlying arthritis and instability.
 
After conservative treatment failed to bring improvement to her injuries, it was decided that Ms. Gibbs would undergo surgery.  On September 27, 2011, she underwent a right DeQuervains release and a right carpal tunnel release.  Following this procedure, Ms. Gibbs underwent a period of physical therapy and was able to return to full duty on November 21, 2011.
 
She lost a total of 120 work days for the second injury.
 
Ms. Gibbs filed two Applications for Adjustment of Claim with the IWCC.  Subject to approval of the IWCC, both claims can be settled for a total amount of $36,595.84, which represents 25% loss of use of a right hand and 5% loss of use of a right thumb.  This settlement would close out any future medical exposure on these claims.
 
The General Counsel believes this settlement is in the best interest of the District and requests payment of that sum be approved and he be authorized to execute such documents as may be necessary to effect the settlement.
