Dear Sir:

 

Larry Williams is a Computer Systems Administrator at the Main Office Building. On November 13, 2013, he sustained an injury to his left arm. Mr. Williams was initially seen by his primary doctor on November 14, 2013. Conservative treatment was unsuccessful and he required surgery.  After undergoing surgery on November 21, 2014, his post surgery treatment consisted of physical therapy. Mr. Williams was released to restricted duty on January 5, 2015. He was able to resume his regular work duties on August 24, 2015. 

 

Mr. Williams was paid a total of $8,519.68, which represents 6.4 weeks of lost time benefits.

 

Subject to the approval of the Illinois Workers’ Compensation Commission, this Pro-Se claim can now be settled for $40,000.00 representing approximately 22% loss of a left arm. This settlement will close out future lost time benefits, as well as any future medical benefits, associated with this injury.

 

The Director of Human Resources believes this settlement is in the best interest of the District and requests payment of that sum be approved and she be authorized to execute such documents as may be necessary to effect the settlement.
