Dear Sir:

 

Robert Sizemore was a Police Officer working at the Calumet Water Reclamation Plant. Mr. Sizemore has since retired from the District effective August 1, 2022. On August 17, 2021, the employee reported that he stepped on loose concrete while walking on the outside of the main gate at the Calumet plant injuring his right leg as a result. He went to the industrial clinic on August 17, 2021, for treatment to his right knee which was initially treated conservatively with medications and an injection. The employee failed to improve from conservative treatment and on November 23, 2021 he underwent a procedure on his right leg at which time he was placed on Police Duty pay. Following his surgery, he underwent a course of physical therapy. He was able to return to work on January 4, 2022.      

 

He was paid Police Duty pay during his time off from work.    

 

Subject to the approval of the IWCC, this Pro-Se case can be settled for $42,310.52, representing approximately 21% loss of use of the right leg. This settlement will close out any future lost time benefits as well as any future medical benefits associated with this injury.

 

The Director of Human Resources believes this settlement is in the best interest of the District and requests payment of that sum be approved and she be authorized to execute such documents as may be necessary to effect the settlement.
