Dear Sir:

 

In 2014, Henry Negoda was a Machinist at the Stickney Water Reclamation Plant.  On March 14, 2014, he sustained injuries to his elbows, knees, right ankle and lower back.  Mr. Negoda was initially seen in the industrial clinic on March 14, 2014.  He was initially treated conservatively with medications.  He followed up with a chiropractor and underwent a course of chiropractic treatment.  He did not lose any time off from work as a result of this injury.    

 

A total of $9,544.17 was paid in medical benefits on Mr. Negoda’s claim. 

 

Mr. Negoda filed an Application of Adjustment of Claim with the Illinois Workers’ Compensation Commission (IWCC) through his counsel.  Subject to the approval of the IWCC, this case can now be settled for $11,424.90, representing approximately 3% loss of use of the person as a whole and $600.00 in outstanding medical bills.  This settlement will close out future lost time benefits, as well as any future medical benefits associated with this injury.

 

The Director of Human Resources believes this settlement is in the best interest of the District and requests payment of that sum be approved and she be authorized to execute such documents as may be necessary to effect the settlement.
