Dear Sir:

 

Anthony Blejski worked as an Electrical Mechanic at the Calumet Water Reclamation Plant.  He retired from the District effective September 1, 2024. On October 16, 2023, he reported that he was lifting a bucket of tools to put them in the work truck when he felt a pop in his right shoulder. Mr. Blejski sought treatment at the Industrial Clinic on October 18, 2023, where he was diagnosed with a strain.

 

Mr. Blejski continued to experience pain. On November 20, 2023, he had an MRI, which revealed a full thickness right rotator cuff tear and tendonitis. Mr. Blejski had surgery on December 22, 2023, at which time he was off work. He returned to work on March 11, 2024. 

 

Mr. Blejski was off work again for work conditioning on June 21, 2024. He returned to work on July 16, 2024.

  

Mr. Blejski was paid a total of 14.6 weeks of lost time benefits totaling $20,513.73.

 

Subject to the approval of the IWCC, this Pro-Se case can be settled for $56,367.85, representing approximately 11.0% loss of use of the person as a whole. This settlement will close out any future lost time and medical benefits associated with this injury.

 

The Director of Human Resources believes this settlement is in the best interest of the District and requests payment of that sum be approved and he be authorized to execute such documents as may be necessary to effect the settlement.
