Authority to settle the Workers’ Compensation Claim of Maurice Thomas vs. MWRDGC, Claim 15 WC 4483, Illinois Workers’ Compensation Commission (IWCC), in the sum of $17,500.00, Account 901-30000-601090

Dear Sir:

 

Maurice Thomas is a Maintenance Laborer Class A at the Stickney Water Reclamation Plant. On November 3, 2014, he caught his shoe on a pallet and tripped and fell injuring his left shoulder. The employee sought treatment at the industrial clinic on November 3, 2014 and was disabled from work effective November 4, 2014. He was admitted to the hospital on November 4, 2014 and had a procedure performed on his left shoulder. He was released from the hospital on November 6, 2014. The employee returned to work on November 17, 2014.

 

Mr. Thomas was paid a total of three days by the Pension fund and was paid a total of 1.20 weeks in lost time benefits totaling $1,087.02.

 

Mr. Thomas filed an Application of Adjustment of Claim with the Illinois Workers’ Compensation Commission (IWCC) through his counsel. Subject to the approval of the IWCC, this case can now be settled for a total of $17,500.00, representing approximately 4.76% loss of use of the person as a whole. This settlement will close out future lost time and medical benefits associated with this injury.

 

The Director of Human Resources believes this settlement is in the best interest of the District and requests payment of that sum be approved and she be authorized to execute such documents as may be necessary to effect the settlement.
