Dear Sir:

 

William Collins was an Operating Engineer I at the Kirie Water Reclamation Plant. He retired from the District effective June 1, 2017. On September 21, 2015, he reported that he injured his upper back while moving a scraper flight from a tank door. The employee sought treatment with his Primary Care Physician and was diagnosed with back strain.

 

Mr. Collins lost no time from work for this injury.

 

Mr. Collins also claimed he had a repetitive trauma injury on May 27, 2017.

 

He lost no time from work for this injury.  

 

Mr. Collins filed Applications of Adjustment of Claim with the Illinois Workers’ Compensation Commission (IWCC) through his counsel. Subject to the approval of the IWCC, these cases can be settled for $14,033.87, representing approximately 3% loss of a person as a whole, $500.00 for the repetitive trauma injury, and $2,205.57 for an outstanding Blue Cross Blue Shield lien. This settlement will close out any future lost time and medical benefits associated with these injuries.

 

The Director of Human Resources believes this settlement is in the best interest of the District and requests payment of that sum be approved and he be authorized to execute such documents as may be necessary to effect the settlement.
