Dear Sir:

 

Kristine Wienberg is a Treatment Plant Operator.  On March 17, 2005, while carrying a ladder at the Calumet Water Reclamation Plant, she stepped on uneven ground and twisted her left foot.  She had no initial treatment and continued working.  She had persistent pain and was later diagnosed with an os peroneal fracture and peroneal tendon tear.  On September 20, 2005, Wienberg underwent surgery to excise the os peroneal bone and repair the peroneal tendon.  She completed a physical therapy program and returned to work on November 29, 2005.  She lost 94 days of work.

 

Subject to the approval of the Illinois Workers’ Compensation Commission, this pro se claim can be settled for the sum of $26,405.96, which represents a 30% loss of the left foot.

 

The Attorney requests payment of that sum be approved and he be authorized to execute such documents as may be necessary to effect the settlement.
