Dear Sir:
On November 19, 2020, the Board of Commissioners authorized the Director of Procurement and Materials Management to issue a purchase order for Contract 20-636-11, Furnishing and Delivering Hydrogen Peroxide to Evoqua Water Technologies, LLC, in an amount not to exceed $402,000.00. The contract expired on April 20, 2023.
As of August 24, 2023, the attached list of change orders has been approved.

A decrease in the contract value is requested to close out the contract. The actual chemical usage can vary depending on conditions, and the exact quantity was not known at the time the contract was drafted. This change order is in compliance with the Illinois Criminal Code since the change is due to circumstances not reasonably foreseeable at the time the contract was signed and is in the best interest of the District.
It is hereby recommended that the Board of Commissioners authorize the Director of Procurement and Materials Management to execute a change order to decrease the purchase order for Contract 20-636-11, in an amount of $10,468.32 (approximately 2.9% of the current contract value) from an amount of $366,412.80 to an amount not to exceed $355,944.48.
Funds will be restored to Account 101-69000-623560.
