Title
Final report on emergency Contract 11-700-01, Rebuilding Primary Tank Drive #9 at the North Side Water Reclamation Plant, to A&L Contractors, Inc., Purchase Order 5001267, Account 101-67000-612650
Body

Dear Sir:

On May 27, 2011, the rake arm on primary tank drive #9 tripped an overtorque sensor. An initial investigation revealed that the bearing races of the turntable that supports the rake arms failed. In order to determine the exact cause of the failure, the entire drive required dismantling. Due to the costly precision components involved, an operating emergency was declared in an amount not to exceed $50,000.00.

A&L Contractors previously performed similar work on primary tank drive #10 at the North Side Water Reclamation Plant and started the repair work on August 29, 2011. After final testing was completed, the tank drive was returned to service on October 28, 2011.

The final cost of the emergency was $41,017.01.

Respectfully submitted, Manju Prakash Sharma, Director of Maintenance and Operations, SES:SO'C:MAG:LSC
