Dear Sir:

 

William Hoskins is a MLAS at the Stickney Water Reclamation Plant. On September 27, 2011, the employee sustained an injury to his back. He was initially seen at the Industrial clinic on September 27, 2011. The employee was disabled effective September 28, 2011. His treatment consisted of physical therapy and medication. He was able to return to work on December 21, 2011.

 

On May 1, 2013, he sustained another lower back injury. He was initially seen at the Industrial clinic on May 2, 2013.  Mr. Hoskins was disabled effective May 3, 2013. His treatment consisted of medication and physical therapy. He was able to return to work on June 16, 2013.

 

Mr. Hoskins was paid a total of $11,210.58 in lost time benefits, which represents 12.1996 weeks of lost time benefits for the injury that occurred on September 27, 2011.

 

Mr. Hoskins was paid a total of $5,954.89 in lost time benefits, which represents 6.4 weeks of lost time benefits for the injury that occurred on May 1, 2013.

 

On January 8, 2014, he sustained another lower back injury. This injury only required conservative treatment. There was no lost time involved.

 

Mr. Hoskins filed multiple Applications of Adjustment of Claims with the Illinois Workers’ Compensation Commission (IWCC) through his counsel. Subject to the approval of the IWCC, these cases can now be settled for $24,146.74, representing approximately 6.69% loss of a person as a whole. This settlement will close out any future lost time or medical benefits associated with these injuries.

 

The Director of Human Resources believes this settlement is in the best interest of the District and requests payment of that sum be approved and she be authorized to execute such documents as may be necessary to effect the settlement.
