Dear Sir:

 

Timothy Moore is an Electrical Mechanic at the Calumet Water Reclamation Plant. On August 30, 2018 he reported multiple injuries including bilateral hip/leg contusions, right elbow fracture and laceration, right hand contusion with foreign body lodged in soft tissue, and hearing loss resulting from a building explosion at the Calumet Plant. He was taken to the University of Chicago Hospital by ambulance on August 30, 2018. X-rays and other diagnostic tests were performed which revealed that he incurred a right elbow fracture with a laceration along with contusions to both hips and legs. Further testing indicated some hearing loss. He was disabled from work effective August 31, 2018. He treated conservatively with injections, medications and a course of physical therapy. He was able to return to work on November 6, 2018.  

 

Mr. Moore was paid a total of 9.40 weeks in lost time benefits totaling $11,939.33.

 

Mr. Moore filed an Application of Adjustment of Claim with the Illinois Workers’ Compensation Commission (IWCC) through his counsel.

 

This claim went to trial on March 12, 2020 at the Illinois Workers’ Compensation Commission (IWCC) and a decision was rendered on June 8, 2020. The Arbitrator awarded the employee a total of $70,611.36 in permanency benefits representing 5% loss of the person as a whole, 17% loss of use of the right arm, 5% loss of use of the right leg, and 8 weeks for disfigurement.

 

The Director of Human Resources believes that payment of this award is in the best interest of the District and requests payment of that sum be approved and she be authorized to execute such documents as may be necessary to effect the award.
