Authority to settle the Workers’ Compensation Claims of David Zintak vs. MWRDGC, Claim 13 WC 037434 and Claim 16 WC 006702, Illinois Workers’ Compensation Commission (IWCC), in the sum of $39,920.76, Account 901-30000-601090

Dear Sir:
David Zintak is a Pollution Control Technician I at the Stickney Water Reclamation Plant. On March 12, 2012, he sustained an injury to his right shoulder. Mr. Zintak was initially seen by the doctor at the industrial medical clinic on March 12, 2012. The employee’s treatment consisted of physical therapy, medication, and injections. Mr. Zintak did not lose any time off from work as a result of the injury.
Mr. Zintak was injured again on January 19, 2016 when he sustained a new injury to his left index finger. He was initially seen at the industrial medical clinic on January 19, 2016. Mr. Zintak required some stitches and conservative care as a result of his injury. He did not lose any time off from work as a result of this injury.
Mr. Zintak filed two Applications of Adjustment of Claims with the Illinois Workers’ Compensation Commission (IWCC) through his counsel. Subject to the approval of the IWCC, these cases can now be settled for a total of $39,920.76, representing approximately 11% loss of the Person as a Whole and approximately 10% loss of use of the left index finger. These settlements will close out future lost time benefits, as well as any future medical benefits associated with these injuries.
The Director of Human Resources believes this settlement is in the best interest of the District and requests payment of that sum be approved and she be authorized to execute such documents as may be necessary to effect the settlement.
