Dear Sir:

 

Vernon Hart is a Pipefitter at the Calumet Water Reclamation Plant. On April 19, 2021, the employee reported that while stepping down from a stool his right foot got caught on the edge of a pallet causing a twisted right ankle. The employee sought medical treatment with his personal physician on April 19, 2021. The employee was disabled effective April 20, 2021. He saw an orthopedic specialist on April 27, 2021, where he was advised he had a right ankle chip/avulsion fracture and treated with a CAM boot, medications, and physical therapy. He was able to return to work on May 17, 2021.     

 

He was paid a total of 3.80 weeks of lost time benefits totaling $5,129.23.    

 

Subject to the approval of the IWCC, this Pro-Se case can be settled for $10,918.42, representing approximately 7.5% loss of use of the right foot. This settlement will close out any future lost time benefits as well as any future medical benefits associated with this injury.

 

The Director of Human Resources believes this settlement is in the best interest of the District and requests payment of that sum be approved and she be authorized to execute such documents as may be necessary to effect the settlement.
