Dear Sir:
Marc Jones is a Truck Driver who works at the Kirie Water Reclamation Plant. On February 17, 2021, he reported sustaining an injury to his left arm and shoulder while exiting his truck, after slipping on snow and falling to the ground striking his left arm and shoulder. The employee was seen on February 17, 2021, at the industrial clinic at which time he was disabled from work. An MRI of his left shoulder on April 8, 2021, revealed a tear of his rotator cuff. On April 26, 2021, Mr. Jones had his left rotator cuff surgically repaired followed by a course of physical therapy. The employee was able to return to work on July 12, 2021.
Mr. Jones was paid a total of 20.2 weeks of lost time benefits totaling $21,008.00.
Subject to the approval of IWCC, this Pro-Se case can be settled for $65,379.75, representing approximately 15.0% loss of person as a whole. This settlement will close out future lost time and medical benefits associated with this injury.
The Director of Human Resources believes this settlement is in the best interest of the District and requests payment of that sum be approved and he be authorized to execute such documents as may be necessary to effect the settlement.
