Dear Sir:

 

Thameena Ali-Mathari was a Maintenance Laborer A Shift at the time of this reported accident working at the Stickney Water Reclamation Plant. On September 16, 2021, she reported that while in the process of collecting sludge samples, she was unable to stop her motorized cart and it collided with a pipe storage rack. The employee went to the industrial clinic on September 16, 2021, for injuries to her right leg.  She was disabled from work effective September 17, 2021. She was treated conservatively with medication and physical therapy. She returned to work on September 27, 2021.

 

Ms. Ali-Mathari was paid 1.20 weeks of lost time benefits totaling $678.57.         

 

Ms. Ali-Mathari filed an Application of Adjustment of Claim with the Illinois Workers’ Compensation Commission (IWCC) through her counsel. Subject to the approval of the IWCC, this case can now be settled for a total of $10,073.93, representing approximately 5% loss of use of the right leg. This settlement will close out future lost time and medical benefits associated with this injury.

 

The Director of Human Resources believes this settlement is in the best interest of the District and requests payment of that sum be approved and he be authorized to execute such documents as may be necessary to effect the settlement.
