Dear Sir:

 

Charles Bravo is an Electrical Operator I at the Calumet Plant.  On August 9, 2007, he was racking breakers when a defective breaker got stuck.  As Mr. Bravo tried to free it, he heard a pop in his left shoulder.  He initially visited his personal doctor who diagnosed a shoulder strain.  An MRI showed an indention of the rotator cuff tendon, narrowing of the subacromial space and secondary manifestations of impingement. Treatment included physical therapy and subacromial injections.  An EMG/NCV showed some degree of brachial plexus strain and a form of thoracic outlet syndrome.  No surgery was recommended; Mr. Bravo underwent work conditioning and returned to full duty work on 1/18/07.  He lost 84 days of work.

 

Subject to the approval of the Illinois Workers’ Compensation Commission, this pro se claim can be settled for the sum of $13,683.59 which represents an 8.5% loss of use of the left arm.

 

The General Counsel requests payment of that sum be approved and he be authorized to execute such documents as may be necessary to effect the settlement.
