Dear Sir:
Matthew Dillon was an Electrical Mechanic at the Calumet Water Reclamation Plant when he was initially injured on February 20, 2018. He sustained injuries to his right shoulder after falling off a ladder while trying to break his fall with his right arm. He was initially seen at the industrial clinic on February 20, 2018. He initially received conservative treatment consisting of physical therapy and medication, but his condition failed to improve. On April 19, 2018, he underwent a procedure on his right shoulder. He was disabled from work effective April 19, 2018. Following his procedure, the employee received a course of physical therapy. He was able to return to work on July 30, 2018.
Mr. Dillon sustained a second injury to his head and left middle finger after he was involved in a building explosion at the Calumet Plant on August 30, 2018. He was taken to Northwestern Hospital by ambulance on August 30, 2018. X-rays and other diagnostic tests were performed for his injuries. He was diagnosed with a fractured left middle finger and a mild concussion. He was disabled from work effective August 31, 2018 and was released to return to restricted duty work on September 5, 2018. He went back to Ingalls industrial clinic on September 5, 2018 and was disabled for September 6, 2018 but was able to return to regular work on September 7, 2018.
Mr. Dillon was disabled for the period April 19, 2018 through July 29, 2018 for his February 20, 2018 injury and was paid a total of $18,067.16 in lost time benefits totaling 14.40 weeks. For his injury on August 30, 2018, he was disabled for the period August 31, 2018 through September 4, 2018 and was paid three days of lost time by the Pension fund. For the September 6, 2018 lost day he was paid a total of $254.03 in lost time benefits totaling .20 weeks.
Mr. Dillon filed on his own an Application for Adjustment of Claim with the Illinois Workers’ Compensation Commission (IWCC) for his February 20, 2018 injury. Subject to the approval of the IWCC, these two claims can now be settled for $51,850.68. This settlement represents 11.5% person as a whole for the right shoulder injury, 7.5% loss of use of the left middle finger, and 1% person as a whole for the injury to head. This settlement closes out any future lost time and future medical benefits associated with these injuries.
The Director of Human Resources believes this settlement is in the best interest of the District and requests payment of that sum be approved and she be authorized to execute such documents as may be necessary to effect the settlement.
