Dear Sir:

 

Thomas Collins is a Truck Driver at the Calumet Water Reclamation Plant.  On December 11, 2009, while exiting his truck, he slipped on ice and twisted his right knee.  He went to Ingalls Occupational Health and was diagnosed with a right knee sprain.  An MRI revealed a lateral meniscus tear.  Mr. Collins underwent surgery on January 26, 2010, that included a partial lateral meniscectomy, medial compartment abrasion arthroplasty, and extensive synovectomy.  He completed a physical therapy program and returned to work on March 15, 2010.  Mr. Collins lost 33 days of work.

 

Subject to the approval of the Illinois Workers’ Compensation Commission, this claim can be settled for the sum of $30, 012.00, which represents a 21% loss of the right leg.

 

The General Counsel believes this is an equitable settlement to conclude this matter and requests payment of that sum be approved and he be authorized to execute such documents as may be necessary to effect the settlement.
