Dear Sir:

 

Andrew Durden was a Maintenance Laborer A at the Calumet Water Reclamation Plant at the time of his accident. He has since retired from the District effective December 31, 2017.  On September 2, 2016, the employee reported he sustained an injury to his lower back while riding a lawn mower and hitting bumps. He was seen at the industrial clinic on September 2, 2016. The employee treated conservatively with medications, physical therapy and steroid injections. The doctor placed the employee on restricted hours October 10, 2016, through October 12, 2016, and then placed him on disability effective October 13, 2016. The employee was paid lost time benefits through October 26, 2016. 

 

Mr. Durden missed 2.0 weeks of work and he worked 3 additional days with restricted hours.  He was paid $1,933.35 in lost time benefits.      

 

Mr. Durden filed an Application of Adjustment of Claim with the Illinois Workers’ Compensation Commission (IWCC) through his counsel. Subject to the approval of the IWCC, this case can now be settled for a total of $14,697.71, representing approximately 3.79% loss of use of the person as a whole. This settlement will close out future lost time and medical benefits associated with this injury.

 

The Director of Human Resources believes this settlement is in the best interest of the District and requests payment of that sum be approved and she be authorized to execute such documents as may be necessary to effect the settlement.
