Dear Sir:
Kevin Mathieu is a Pipefitter at the Stickney Water Reclamation Plant. On February 16, 2022, he reported that he was using a 10-pound sledgehammer to remove rusted bolts from a flange when he experienced extreme pain in his right shoulder. Mr. Mathieu sought treatment at the Industrial Clinic on February 16, 2022, and was diagnosed with a right shoulder strain.
Mr. Mathieu had an MRI in March 2022 which revealed mild AC degenerative changes, cuff tendonitis, and a SLAP tear posterior labrum. An anteroinferior labral tear was also suspected. He was taken off work on March 24, 2022, and was treated conservatively with injections and physical therapy. He was able to return to work on June 29, 2022.
Mr. Mathieu was paid a total of 13.8 weeks of lost time benefits totaling $19,005.12.
Subject to the approval of the IWCC, this Pro Se case can be settled for $23,427.75, representing approximately 5% loss of a person as a whole. This settlement will close out any future lost time and medical benefits associated with this injury.
The Director of Human Resources believes this settlement is in the best interest of the District and requests payment of that sum be approved and he be authorized to execute such documents as may be necessary to effect the settlement.
