Authority to settle the Workers’ Compensation Claim of Charmion Soto vs. MWRDGC, Claim 15 WC 30402, Illinois Workers’ Compensation Commission (IWCC), in the sum of $15,104.40, Account 901-30000-601090

Dear Sir:
Charmion Soto is a Fireman/Oiler at the Stickney Water Reclamation Plant. On July 15, 2015, she sustained an injury to her groin area and her right shoulder. Ms. Soto was initially seen by the doctor at the industrial medical clinic on July 15, 2015. She underwent a conservative treatment consisting of physical therapy and medication. The employee did not lose any time from work for this injury.
Ms. Soto filed an Application of Adjustment of Claim with the Illinois Workers’ Compensation Commission (IWCC) through her counsel. Subject to the approval of the IWCC, this case can now be settled for $15,104.40, representing approximately 4% loss of use of the person as a whole. This settlement will close out future lost time benefits, as well as any future medical benefits associated with this injury.
The Director of Human Resources believes this settlement is in the best interest of the District and requests payment of that sum be approved and she be authorized to execute such documents as may be necessary to effect the settlement.
