Dear Sir:

 

Michael Osborne is an Operating Engineer II at the Stickney Water Reclamation Plant.  On March 27, 2012, he sustained an injury to his left knee.  Mr. Osborne was initially seen by the doctor at the industrial medical clinic on March 27, 2012.  He was initially disabled from work effective April 2, 2012. He then returned to work on April 9 2012.  He was disabled from work again effective May 8, 2012.  The employee’s treatment consisted of physical therapy, medication, and injections.  Mr. Osborne returned to work on July 23, 2012.  

 

Mr. Osborne was injured again on July 3, 2013.  He came in contact with sludge and developed a rash on his arm.  He was initially seen at the industrial medical clinic on July 3, 2013.  Mr. Osborne was disabled from work effective August 14, 2013 and returned to work on August 26, 2013.  Mr. Osborne required topical treatment for his rash condition.

 

Mr. Osborne was paid a total of $13,788.89 in lost time benefits for the March 27, 2012 injury for a total of 11.80 weeks.  He was paid a total of two weeks in amount of $2,372.56 for the July 3, 2013 injury.   

 

Mr. Osborne filed two Applications of Adjustment of Claims with the Illinois Workers’ Compensation Commission (IWCC) through his counsel.  Subject to the approval of the IWCC, these cases can now be settled for a total of $13,500.00, representing approximately 9.02 % loss of the left leg. This settlement will close out future lost time benefits, as well as any future medical benefits associated with these injuries.

 

The Director of Human Resources believes this settlement is in the best interest of the District and requests payment of that sum be approved and she be authorized to execute such documents as may be necessary to effect the settlement.
