Dear Sir:

 

Earl Ebers, Jr.  was an Operating Engineer I at the Calumet Water Reclamation Plant at the time of his accident. He has since retired from the District effective July 31, 2020.  On June 17, 2018, the employee reported he sustained a laceration to his left hand while cleaning suction grating for air handling unit for the digestor office.  He was seen at the industrial clinic on June 17, 2018. The doctor placed the employee on disability effective June 28, 2018. The employee underwent surgery on July 3, 2018. The employee returned to work on August 27, 2018. 

 

Mr. Ebers missed a total of 8.80 weeks and was paid $11,451.64 in lost time benefits.      

 

Mr. Ebers filed an Application of Adjustment of Claim with the Illinois Workers’ Compensation Commission (IWCC) through his counsel. Subject to the approval of the IWCC, this case can now be settled for a total of $13,519.94, representing approximately 45% loss of use of the left middle finger.   This settlement will close out future lost time and medical benefits associated with this injury.

 

The Director of Human Resources believes this settlement is in the best interest of the District and requests payment of that sum be approved and she be authorized to execute such documents as may be necessary to effect the settlement.
