COMMITTEE ON JUDICIARY

Mr. Brian A. Perkovich, Executive Director

Title

Authority to settle the Workers’ Compensation Claim of Keefe Ammons vs. MWRDGC, Claim number 19WC034152, Illinois Workers’ Compensation Commission (IWCC), in the sum of $20,917.25, Account 901-30000-601090

Body

Dear Sir:

Keefe Ammons was a Maintenance Laborer A at the Stickney Water Reclamation Plant at the time of his accident. He has since retired from the District effective April 30, 2020.  On November 13, 2019, the employee reported he sustained injuries to low back, right ankle, right shoulder, and right wrist after falling off the bed of a truck while unloading bags of salt. He was seen at the industrial clinic on November 13, 2019. The doctor placed the employee on disability effective November 14, 2019. The employee treated conservatively with medications and physical therapy. The employee returned to work on February 28, 2020.

Mr. Ammons missed a total of 15.20 weeks and was paid $15,784.25 in lost time benefits.

Mr. Ammons filed an Application of Adjustment of Claim with the Illinois Workers’ Compensation Commission (IWCC) through his counsel. Subject to the approval of the IWCC, this case can now be settled for a total of $20,917.25, representing approximately 5% loss of use of the person as a whole.    This settlement will close out future lost time and medical benefits associated with this injury.

The Director of Human Resources believes this settlement is in the best interest of the District and requests payment of that sum be approved and she be authorized to execute such documents as may be necessary to effect the settlement.

Requested, Beverly K. Sanders, Director of Human Resources, BKS:RAJ:RG
