Dear Sir:

 

Timothy Portal is a Pipefitter, working at the Calumet Water Reclamation Plant. On July 17, 2018, the employee reported that he was pulling on a rope attached to a six-inch hose when the clamp got caught on the inside of a manhole resulting in a significant strain of the right shoulder. The employee sought medical treatment immediately at the industrial clinic on July 17, 2018. He was treated conservatively with physical therapy and medications. The conservative treatment failed, and the employee was placed on disability effective October 30, 2018. On October 31, 2018, he underwent surgery to repair his right shoulder. Following the surgery, the employee underwent a course of physical therapy. He was able to return to work on February 25, 2019.  

 

He was paid a total of 16.80 weeks of lost time benefits totaling $21,333.41. 

 

Subject to the approval of the IWCC, this Pro-Se case can be settled for $51,391.60, representing approximately 12.6% loss of use of the person as a whole. This settlement will close out any future lost time benefits as well as any future medical benefits associated with this injury.

 

The Director of Human Resources believes this settlement is in the best interest of the District and requests payment of that sum be approved and she be authorized to execute such documents as may be necessary to effect the settlement.
