Dear Sir:

 

Gary Dorigan is an Operating Engineer I at the Calumet Water Reclamation Plant. On June 22, 2019, the employee reported that while walking down a ramp his boot allegedly got caught on rubber matting causing him to fall injuring his left leg. The employee was initially seen at the industrial clinic on June 22, 2019. The employee was initially disabled effective June 23, 2019, before returning to work on September 11, 2019. On November 19, 2019, the employee underwent a surgical procedure on his left leg. He was disabled from work again at this time before returning to work on March 4, 2020.     

 

Mr. Dorigan was not paid any lost time benefits.      

 

Mr. Dorigan filed an Application of Adjustment of Claim with the Illinois Workers’ Compensation Commission (IWCC) through his counsel. Subject to the approval of the IWCC, this case can now be settled for a total of $17,498.20, representing approximately 10% loss of use of the left leg. This settlement will close out future lost time and medical benefits associated with this injury.

 

The Director of Human Resources believes this settlement is in the best interest of the District and requests payment of that sum be approved and she be authorized to execute such documents as may be necessary to effect the settlement.
