Report on the Settlement of Workers’ Compensation Claims and Miscellaneous Claims under $10,000.00

Dear Sir:

 

The following matters were settled upon the recommendation of the Director of Human Resources and the approval of the Executive Director:

 

1.      Settlement of General Liability Claim of Alysia Burns, Claim No: A/P 1538, in the amount of $5,476.85.

       While driving down 162
nd Street in South Holland, IL on January 24, 2023, Ms. Burn’s allegedly drove her personal vehicle over an unsecured manhole owned and maintained by the District causing damage to the vehicle resulting in a total loss. This settlement satisfies the total value of the vehicle and vehicle transportation expense.

2.       Settlement of Auto Liability Claim of Kulsum Sikander, Claim No: A/P 1534, in the amount of $6,178.06.

 

While driving through the intersection of Devon and Caldwell Avenues in Chicago, IL on October 13, 2022, Mr. Sikander’s personal vehicle was allegedly struck and damaged on the rear end by another vehicle who was also struck and damaged by a District owned vehicle. This settlement satisfies the total damages sought to repair the vehicle.

3.       Settlement of Workers’ Compensation Claim of Kerry Jackson, Claim Case No. W003395537, in the amount of $8,717.30.

       Ms. Kerry Jackson, a Maintenance Laborer A at the Kirie WRP, alleged an April 2, 2021 work related injury to her left shoulder while she pulled a fire house. The industrial clinic determined she sustained injuries to her left shoulder and cervical area. She was off work from April 5, 2021, through June 14, 2021, and she returned to work on June 15, 2021. Settlement in the amount of $8.717.30 represents 2% loss of use of the person as a whole and will close out any future lost time benefits as well as any future medical benefits.

4.      Settlement of Workers’ Compensation Claim of McGill Dillon, Claim Case No. 23 WC 010829, in the amount of $2,436.49.

       Mr. McGill Dillon, an Electrical Mechanic at the Stickney WRP, alleged a September 15, 2021 work related injury to his right foot when a machine roller fell off of a cart he was pushing and landed on his right foot. The industrial clinic determined he fractured his right fifth toe. He was off work from September 15, 2021, through January 5, 2022, and he returned to work on January 6, 2022. Settlement in the amount of $2,436.49 represents 20% loss of use of his right fifth toe and will close out any future lost time benefits as well as any future medical benefits.

5.      Settlement of Workers’ Compensation Claim of Matthew Dillon, Claim Case No. W003712125, in the amount of $1,827.36.

 

Mr. Matthew Dillon, an Electrical Mechanic Sub-Foreman at the Calumet WRP, alleged a March 23, 2022 work related injury to his right foot while helping an electrician move an air conditioner which landed on his right foot. The industrial clinic determined he fractured his right fourth toe. Mr. Dillon did not lose anytime from work. Settlement in the amount of $1,827.36 represents 15% loss of use of his right fourth toe and will close out any future lost time benefits as well as any future medical benefits.
