Authority to settle the Illinois Workers’ Compensation Claim of James Keeley vs. MWRDGC, Claim Number 23WC030460 in the sum of $18,500.00, Account 901-30000-601090

Dear Sir:
James Keeley is Pipefitter who was working at the Stickney Water Reclamation Plant at the time of his accident. On August 23, 2022, the employee reported that he injured his groin area while carrying an extension ladder. He went to the industrial clinic on August 23, 2022, at which time it was determined that he had sustained a hernia. The employee underwent a procedure to repair his hernia on September 21, 2022, at which time he was disabled from work. He was able to return to work on October 19, 2022.
The employee was paid 4 weeks of lost time benefits totaling $5,570.84.
Subject to the approval of the IWCC, this Pro-Se case can be settled for $18,500.00, representing approximately 3.71% loss of use of the person as a whole. This settlement will close out any future lost time benefits as well as any future medical benefits associated with this injury.
The Director of Human Resources believes this settlement is in the best interest of the District and requests payment of that sum be approved and he be authorized to execute such documents as may be necessary to effect the settlement.
