Dear Sir:

 

David Mica is a Pipefitter at the Stickney Water Reclamation Plant. On August 25, 2022, he reported that he was walking down the stairs in the West Digester carrying a bucket of tools, when he slipped and fell on his tailbone. Mr. Mica sought treatment at the Industrial Clinic on August 25, 2022, where he was diagnosed with a strain. He treated conservatively with physical therapy.

 

Mr. Mica continued to have pain. On September 2, 2022, he had X-rays, which revealed mild disk degeneration at L5-S1, however, the doctor determined his injury to be muscular in nature. Mr. Mica was taken off work effective September 8, 2022. He returned to work on October 6, 2022. 

 

Mr. Mica was paid a total of 4.0 weeks of lost time benefits totaling $5,571.24.

 

Subject to the approval of the IWCC, this Pro-Se case can be settled for $11,726.74, representing approximately 2.35% loss of use of the person as a whole. This settlement will close out any future lost time and medical benefits associated with this injury.

 

The Director of Human Resources believes this settlement is in the best interest of the District and requests payment of that sum be approved and he be authorized to execute such documents as may be necessary to effect the settlement.
