Dear Sir:

 

Frank Holdman is a Maintenance Laborer A at the Stickney Water Reclamation Plant. On July 21, 2021, he reported that, while he was cleaning, he knocked over a bucket of mop water, slipped and fell hurting his left leg. He went to Adventist Hinsdale Hospital. On July 22, 2021, Mr. Holdman had surgery to repair a left supracondylar distal fracture of the left femur. Mr. Holdman was off work starting on July 22, 2021, and returned to work on February 7, 2022.  

  

Mr. Holdman was paid a total of 28.4 weeks of lost time benefits totaling $30,581.12.

 

Mr. Holdman filed an Application of Adjustment of Claims with the Illinois Workers’ Compensation Commission (IWCC) through his counsel. Subject to the approval of the IWCC, this case can now be settled for a total of $40,295.73, representing approximately 20.0% loss of use of a leg. This settlement will close out future lost time, and medical benefits associated with this injury.

 

The Director of Human Resources believes this settlement is in the best interest of the District and requests payment of that sum be approved and he be authorized to execute such documents as may be necessary to effect the settlement.
