Dear Sir:

 

Frank Pulido is a Pipefitter working at the Stickney Water Reclamation Plant. On February 25, 2022, he reported that he slipped while walking downstairs and injured his back. Mr. Pulido sought treatment at the Industrial Clinic on February 28, 2022. He was diagnosed with rib fractures and contusions of the thoracic spine. Mr. Pulido was treated conservatively with medication and restricted duty. He had no lost time for this injury.

 

On July 11, 2023, Mr. Pulido reported that he was pulling on pipe wrenches when he injured his left shoulder. He initially sought treatment at the Industrial Clinic on July 20, 2023, and was diagnosed with shoulder strain. He was treated conservatively with physical therapy.

 

Mr. Pulido’s condition did not improve, and he sought treatment with an Orthopedic Surgeon. He was taken off work on July 25, 2023, and returned to work on August 25, 2023.

 

On January 10, 2024, Mr. Pulido underwent an orthoscopic rotator cuff repair and subacromial decompression. He was off work on January 8, 2024, and returned to work on May 9, 2024.

 

Mr. Pulido was paid 22.2 weeks of lost time benefits totaling $31,498.82.

 

Mr. Pulido filed Applications of Adjustment of Claim with the Illinois Workers’ Compensation Commission (IWCC) through his counsel. Subject to the approval of the IWCC, these cases can be settled for $79,427.43 representing approximately 15.5% loss of a person. This settlement will close out any future lost time and medical benefits associated with these injuries.

 

The Director of Human Resources believes this settlement is in the best interest of the District and requests payment of that sum be approved and he be authorized to execute such documents as may be necessary to effect the settlement.
