Dear Sir:
Cristobal Gutierrez is a Structural Ironworker at the Stickney Water Reclamation Plant. On November 22, 2023, he reported that after swinging a four-pound hammer against a steel plate, he had pain in his right wrist. Mr. Gutierrez sought treatment at the Industrial Clinic on November 22, 2023, and was diagnosed with tendonitis of the extensor tendon.
An MRI revealed a Triangular Fibrocartilage Complex Tear and De Quervain Tenosynovitis in his right wrist. Mr. Gutierrez was off work effective January 1, 2024, and returned to work on January 8, 2024. His symptoms continued. He had surgery on February 9, 2024, and was again off work at that time. He was able to return to work on February 26, 2024.
Mr. Gutierrez was paid a total of 3.2 weeks of lost time benefits totaling $4,810.52.
Subject to the approval of the IWCC, this Pro Se case can be settled for $31,514.75, representing approximately 15% loss of use of the hand. This settlement will close out any future lost time and medical benefits associated with this injury.
The Director of Human Resources believes this settlement is in the best interest of the District and requests payment of that sum be approved and he be authorized to execute such documents as may be necessary to effect the settlement.
