Dear Sir:
Ebonie Lewis Bradford was a Human Resources Analyst at the Main Office Building Annex (MOBA). She separated from the District effective April 19, 2023. On June 7, 2019, Ms. Lewis Bradford reported that she moved her computer monitor and it fell onto her right foot. She sought treatment at the Emergency Room and was diagnosed with a right foot contusion.
She continued to have pain. An MRI revealed a fracture on the 4th metatarsal base and a fracture of the 5th metatarsal. She was off work June 12, 2019, and returned to work on July 2, 2019. Ms. Lewis Bradford was off again July 12, 2019, and returned on July 15, 2019.
Ms. Lewis Bradford continued to have pain and was off work again October 14, 2019, and returned to work on November 11, 2019.
Ms. Lewis Bradford was paid 7.0 weeks of lost time benefits totaling $6,812.36.
Ms. Lewis Bradford filed an Application of Adjustment of Claim with the Illinois Workers’ Compensation Commission (IWCC) through her counsel. Subject to the approval of the IWCC, this case can be settled for $30,000.00 representing approximately 17.0% loss of use of the foot and outstanding medical bills. This settlement will close out any future lost time and medical benefits.
The Director of Human Resources believes this settlement is in the best interest of the District and requests payment of that sum be approved and he be authorized to execute such documents as may be necessary to effect the settlement.
