Dear Sir:

 

Silvana Chimera is a Stores Specialist at the Stickney Water Reclamation Plant. On July 6, 2020, she reported that while moving materials around the storeroom she hurt her right shoulder. Ms. Chimera did not seek treatment.

 

On September 29, 2020, Ms. Chimera reported that while retrieving office supplies, she tripped over a cord to a dehumidifier and fell on her left side. She treated at the Industrial clinic and was diagnosed with left hip, left rib, and left shoulder contusions, and low back strain. She treated conservatively with physical therapy.

 

Ms. Chimera sought treatment with her Orthopedic for her left hip and left shoulder. She received injections to both her left hip and left shoulder. An MRI revealed tears in both shoulders. 

 

Ms. Chimera was not paid any lost time benefits.

 

Ms. Chimera filed Applications of Adjustment of Claim with the Illinois Workers’ Compensation Commission (IWCC) through her counsel. Subject to the approval of the IWCC, these cases can be settled for $108,966.25 representing approximately 25% loss of a person as a whole. This settlement will close out any future lost time and medical benefits.

 

The Acting Director of Human Resources believes this settlement is in the best interest of the District and requests payment of that sum be approved and he be authorized to execute such documents as may be necessary to effect the settlement.
